Who can request a birth certificate?

Births less than 100 years old are confidential and may only be issued to:

  • Registrant if of legal age
  • Parent listed on the Birth Record
  • Legal representative of one of the above persons
  • Upon receipt of registrants death record, a birth certificate marked Deceased may be issued to the spouse, child, grandchild, or sibling, if of legal age. Please notate Registrant Deceased on the application and include a photocopy of the death certificate.


If applicant is not one of the above, the application must be accompanied with a notarized Affidavit to Release a Birth Certificate signed by one of the above, along with any supporting documentation and a copy of valid photo ID of both the person authorizing release and the applicant.

Ordering a Florida Birth Certificate

Any person who willfully and knowingly provides any false information on a certificate, record or report required by Chapter 382, Florida Statutes, or on an application or affidavit, or who obtains confidential information from any Vital Record under false or fraudulent purposes, commits a felony of the third degree, punishable as provided in Chapter 775, Florida Statutes.

Homeless Youth Birth Certificates

A homeless youth birth certificate is an official legal document. It expires six months from the date issued. You may request this certificate at no cost from any Minnesota vital records office if you are homeless, 24 years old or younger, and born in Minnesota.

You must complete the Homeless Youth Birth Certificate Application and provide identification. You may provide acceptable identification or submit a Statement to Identify or an original or a copy of a signed Affidavit of Homeless Status form. The Affidavit of Homeless Status is valid for six months after it is signed.

How Can I Use A Birth Record Outside The United States

If you want to use a certified document in another country, you may need an apostille from the Secretary of State. For example, a birth certificate issued in the state of Washington that you want to use in another country, may require an apostille.

Who May Apply For A Birth Certificate

The person requesting the certificate must meet one of the following eligibility standards:

  • Subject of the Record
  • Person Acting the Subjects Best Interest
  • Parent named on the record
  • Spouse, Step Parent, Grandparent or Adult Child/Grandchild of the Subject along with a signed authorization from the subject and a copy of the subjects ID if the subject is age 18 or older
  • Attorney for the Subject, Adoptive Parents, Representative with Power of Attorney Document, Legal Guardian, or Person with a Court Order

    Puerto Rico to Cancel Birth Certificates Issued Before July 1, 2010

    Special Note: Effective July 1, 2010, Puerto Rico-issued birth certificates dated before July 1, 2010, need to be replaced by October 30, 2010.

    Under a new law in Puerto Rico, all Puerto Rico-issued birth certificates dated before July 1, 2010, will be cancelled as of October 30, 2010, so that new, more secure certificates can be re-issued. Until that date, all birth certificates will remain valid.

    This new law is based on collaboration with the U.S. Department of State and the U.S. Department of Homeland Security to address the fraudulent use of Puerto Rico-issued birth certificates. They have been used illegally to obtain U.S. passports, Social Security benefits and other federal services. The government of Puerto Rico has enacted the new law to combat fraud and protect the identity and credit of all people born in Puerto Rico.

    To protect the identity and privacy of citizens of the District of Columbia, DC Health restricts access to birth certificates that arent in the public record.

    However, you ARE entitled if you are a:

    During your visit, you will be required to complete an identity verification questionnaire at our self- service kiosks.

    If the name on your identification does NOT match the name on the certificate, you must prove evidence of a legal name change. This may include a certified marriage certificate, certified divorce decree or a certified legal name change court order that reflects the history of the changes to the name on the certificate requested.

    If the kiosk is able to authenticate your identity, you will be required to present at least ONE of the following forms of primary photo ID :

    • State-issued drivers license or non-drivers ID card
    • Passport or passport card
    • Military ID Card
    • Law enforcement ID
    • Government employee ID

    If the kiosk is able to authenticate your identity but you do not have a primary photo ID, you will be required to present TWO FORMS of alternate identification from the list below :

    To obtain a copy of a birth certificate you will need to complete the application and mail it to your birth states vital records office. You will also need to prove your identity.

    How Much Does A Certified Copy Of A Birth Certificate Cost

    The fee for the search of a birth certificate is $15.00 and will include the issuance of one certified copy to a qualified applicant if the certificate is on file. Additional copies will be issued at a cost of $15.00 each. The searching fee is not refundable or transferable. All fees expire one year from the receipt date. Online and phone orders are subject to a an additional service fee.
