According to the Department of Health and Human Services, Office of Inspector General, as of 2000 there were more than 6,000 entities issuing birth certificates. The Inspector General report stated that according to the staff at the Immigration and Naturalization Service‘s Forensics Document Laboratory the number of legitimate birth certificate versions in use exceeded 14,000.
Short-form birth certificates and acceptance thereof
In the case of applying for a U.S. passport, not all legitimate government-issued birth certificates are acceptable:
A certified birth certificate has a registrar’s raised, embossed, impressed or multicolored seal, registrar’s signature, and the date the certificate was filed with the registrar’s office, which must be within 1 year of your birth. Please note, some short versions of birth certificates may not be acceptable for passport purposes.
Beginning April 1, 2011, all birth certificates must also include the full name of the applicant’s parent.
The U.S. State Department has paid close attention to abstract certificates from both Texas and California. There have been reports of a high incidence of midwife registration fraud along the border region between Texas and Mexico, and the Texas abstract certificate form does not list the name or occupation of the attendant. The California Abstract of Birth did not include an embossed seal, was no longer considered a secure document, and have not been issued in California since 2001.

To get a birth certificate, you should have the following information ready:
- Birthplace, including the city, county, and hospital if applicable
- Parentsâ full names, including motherâs maiden name

- Provide documentation for court proceedings
- Enter the armed forces
- Get welfare assistance, veteranâs benefits, or other government benefits
- Replace a lost, stolen, or damaged birth certificate
- Get an additional copy or informational copy
- Obtain a delayed birth certificate
Some application forms may present various options, while others ask you to fill in the blank. Requesting a delayed birth certificate means it wasnât filed within a year of the personâs birth. In that case, youâd be requesting the original document.
How Do I Get A Certificate Of Foreign Birth
To place a record on file, complete the application and submit it to Vital Records with $40 and a copy of your photo ID along with the two required documents listed above.

What Is An Heirloom Birth Certificate
The Heirloom Birth Certificate is 8 1/2 x 11 with a chocolate brown and mint green color palette bearing a depiction of a redbud – the official state tree. The certificate will reflect the child’s name, date of birth, gender, place of birth and the names of the parents. The cost for an Heirloom birth certificate is $35.00. This $35 fee will provide for the issuance of both one Heirloom birth certificate and one standard Certified birth certificate.
The same eligibility and identity requirements for any birth certificate also apply to the request for an heirloom certificate. The Heirloom certificate is a commemorative certificate and should not be used for proof of birth or identity for legal purposes.

Born In Boston You Can Pick Up Your Birth Certificate At City Hall
Boston has birth records from 1630 to present day. Birth certificates for recently born babies are available at Boston City Hall about two weeks after the babys paperwork is completed at the hospital.
If the parents werent married when the baby was born, the birth certificate becomes restricted. Restricted means that only those listed on the birth record can get a copy of a restricted birth certificate, and you have to show a valid ID .
COST OF CERTIFICATES
Certificates cost $12 per copy when you buy them in person. We accept cash, credit cards, pinless debit cards, and checks or money orders made payable to the City of Boston.
If you use a credit card or pinless debit card, there is a non-refundable service fee of 2.5% of the total payment, with a $1 minimum. This fee is paid to the card processor and not kept by the City.
If your debit card requires you to enter your pin to process a payment, you CANNOT use it to pay your fee.

You are unable to get a certified birth certificate in the US if you were born in another country and adopted by a US citizen. The country you were born in issued your birth certificate, so you must contact their foreign embassy or consulate. Each country has specific guidelines you must follow in order to obtain your documentation.
Can I Order An Original Birth Certificate
In reality, obtaining your original birth certificate can usually only be done through a court order, by permission, or by death.

Effective January 1, 2013, the Center for Health Statistics may prepare and register a Certificate of Foreign Birth upon receipt of a request from an adoptive parent or an adopted individual 19 years of age or older. A Certificate of Foreign Birth may be put on file for a child who has automatically acquired United States citizenship following a foreign adoption and who possesses a Certificate of Citizenship in accordance with the Child Citizenship Act. What Types Of Birth Certificates Exist
It may shock you to find out that there’s not a singular type of birth certificate. Vital records come in a variety of forms, in the sense that some can be used for certain things while others can’t. When it comes to birth certificates, you’ll first need to differentiate between the following:

The person requesting the certificate must meet one of the following eligibility standards:
- Subject of the Record
- Person Acting the Subjects Best Interest
- Parent named on the record
- Spouse, Step Parent, Grandparent or Adult Child/Grandchild of the Subject along with a signed authorization from the subject and a copy of the subjects ID if the subject is age 18 or older
- Attorney for the Subject, Adoptive Parents, Representative with Power of Attorney Document, Legal Guardian, or Person with a Court Order
Additional documentation may be required demonstrating the requestors authorization to obtain the birth record requested.
Citizens Born Abroad On A Military Base
The birth of a child to American parents on a military base abroad is typically documented the same way as children born to American parents in a foreign country.
These children often have a Consular Report of Birth Abroad issued in their name, and can therefore take the same steps explained above to obtain a copy.
If you were born to American parents on a military base abroad and your parents did not file for a CRBA, you can try contacting the following offices for additional information:
- The hospital where the birth took place.
- The base operator.
- The public affairs office of the appropriate military branch.

Who Can Obtain A Certified Copy Of A Birth Certificate
You are entitled to obtain a certified copy of a birth certificate if you are:
- The person named on the certificate
- The parent named on the birth certificate
- The legal guardian of a minor or adult
- A legal representative of one of these.
Immediate family members and/or their respective legal representative may obtain a certified copy of a birth record if the registrant is deceased. An original, certified copy of the registrant’s death record must be submitted with the request. There are no exceptions to this requirement. Birth certificates become public records after 100 years. Then any person may obtain an uncertified copy of the birth certificate, upon submission of application form and fees.
