How Do I Amend Or Change The Information On A Birth Certificate
The procedures and requirements to change information on a birth certificate depend upon the specific information to be changed and whether any previous changes have been made.
The information contained on birth records is assumed to be correct as it is. However, sometimes there is an error that was not addressed during the registration process . These corrections require you to provide one or more evidences as to what the correct information should have been . These evidences must meet certain regulatory and industry standard requirements.
All evidence submitted is for review purposes only and may be rejected by our office if there is reason to doubt its validity or adequacy to meet those requirements. If the evidence is approved, our office will prepare an affidavit for you to sign and have notarized. After the affidavit is correctly executed the amendment will be applied to the record. If proper evidence is not supplied, the amendment can only be completed upon receipt of a court order.

If You Have A Home Birth
Complete and return the home birth worksheet within one year of the baby’s birth. If you do not return this form within a year of the child’s birth, the parent will need to provide additional evidence about the birth and pay an additional filing fee. More importantly, the birth certificate will be marked delayed. Government authorities question the validity of delayed birth certificates, and this could impact the child for his or her whole life.

Birth records in the IDOH Vital Records office begin with October 1907. Prior to October 1907, records of birth are filed only with the local health department in the county where the birth actually occurred.
Fees are established by law . Each search for a record costs $10.00. The fee is non-refundable. Included in one search is a five-year period the reported year of birth and, if the record is not found in that year, the two years before and after. One certified copy of the record, if found, is included with the search fee. Additional copies of the same record purchased at the same time are $4.00 each. Amendments made to the record are an additional $8.00.

Birth Certificate With Parental Information
A birth certificate with parental information is an extract of information from the original birth registration and includes:
- all of the information included in the birth certificate
- the name and place of birth of the subjects parent
This certificate is recommended for newborns and children under 16 years of age where parental information is required. It can be used for the same identification purposes as a birth certificate with only subject information, but also for:
- a childs passport in Canada
- child health insurance

If You Adopt A Child Born In Rhode Island
- The court hearing the adoption will forward the to the Center for Vital Records. For children adopted in Rhode Island, the Center for Vital Records usually gets the report about three weeks after the court hears the adoption. Access Adoption Records Through Consent
In some states, you may be able to access identifying information through a mutual consent registry. Using these registries, all involved in an adoption can declare what information may be disclosed. Some states may require the consent of both the birth parents and adoptive parents for the release of records. However, the release of information varies by state.
If your state does not maintain a mutual consent registry, there are other ways to obtain records through consent. Public or private agencies can locate birth parents in some states. When an agency contacts birth parents, they can find out identifying information through:
- Confidential Intermediary System The court gives permission to a court-certified confidential intermediary. This permission grants them access to sealed adoption records. They can also contact the birth parents to obtain consent for the release of identifying information.
- Affidavit System – Birth parents can officially file their consent or refusal to be identified or contacted.

Options To Establish Paternity Voluntarily
It’s easiest for an unmarried couple to establish paternity voluntarily. In some states, a man can establish paternity by simply signing a sworn voluntary declaration that the child is his. He should also file the statement of paternity with the Bureau of Vital Statistics.
In other states, like California, a child’s unmarried parents can agree to sign a “Voluntary Declaration of Paternity.” Attending medical providers are legally obligated to give an unmarried mother information on the voluntary declaration of paternity. Death In The Us: How To Get A Certified Copy Of A Death Certificate
Contact the vital records office of the state where the death occurred to learn:

How Can I Use A Birth Record Outside The United States
If you want to use a certified document in another country, you may need an apostille from the Secretary of State. For example, a birth certificate issued in the state of Washington that you want to use in another country, may require an apostille.

Our office has birth records from July 1, 1907 to present for events that occurred in the state of Washington. You can obtain Washington state birth records as certified copies for legal purposes or noncertified informational copies for genealogical or other personal use. The three different birth record products include: birth certificate, Heirloom birth certificate, and noncertified informational copy of a birth record.

If You Adopt A Child Born In A Foreign Country
- Contact the Center for Vital Records for information on creating a certificate of foreign birth.
- Some foreign countries require adopting parents to have an apostille attached to a certified copy of the vital record before adopting a child. If the foreign country requires an apostille, the certified copy of the birth certificate must be personally signed by the State Registrar or the Deputy State Registrar. Contact the Secretary of State’s Notary Division, 401-222-1487, to ask if you need an apostille or certification.
Requirements To Get A Birth Certificate Replacement
Not everyone is able to get a copy of your certified birth certificate. Your documentation is an important form of identification, which is why there are restrictions when it comes to obtaining a copy of your US birth certificate. In order to receive someone elses or your birth certificate, you must:
- Be the individual named on the certificate and be 18 years of age or older.
- Be the parent, guardian or legal representative of the person who is on the certificate.
Those with any other relationship must provide a court order establishing guardianship.
All states allow you to replace birth certificates in person or by mail. Regardless of which method you decide to use to get your birth certificate, you will need to provide specific items, such as:
- A completed birth certificate request form.
- A check or money order for applicable fees.
- A valid form of identification .
- A self-addressed stamped envelope with your request by mail.
If you go in person or mail your request for new documentation without any of the items listed above, you will not get a copy of your birth certificate.
Be sure to have the correct amount of money to pay for a copy of your documentation. Prices vary depending on the state you live in. For example, in Nebraska and Delaware it cost $17 and in Oklahoma the price is $15 to receive a certified copy of your identification.

Born In Boston You Can Order Your Birth Certificate Online
Boston has birth records from 1870 to present day. Birth certificates for recently born babies are available at Boston City Hall about two weeks after the babys paperwork is completed at the hospital. Boston birth records from 1630-1869 require a $10 research fee, more time, and should be ordered by mail.
If the parents werent married when the baby was born, the birth certificate becomes restricted. Only those listed on a restricted record can request it. For online orders, you will need to upload:
- a photocopy of your valid ID , and
- any other required documents when making your request.
Birth certificates cost $14 per copy. That price includes shipping. You will be charged an extra service fee of not more than $0.25, plus 2.15%. That fee goes directly to a third party to pay for the cost of card processing. We have more information about card service fees at the City of Boston.
