The Office of Vital Records does not have a service window send your application and payment .
Sign your application form in front of a notary public before you send your request. The Office of Vital Records returns request forms that do not have a notary signature and stamp or seal. We also return incomplete forms or forms not paid in full at the time of application.
The Office of Vital Records accepts payment by credit card . We also take checks or money orders. Make your check or money order payable to Minnesota Department of Health. We deposit the payments we receive upon receipt.
Birth Certificate Copies: Americans Born Abroad
If you were born to American parents abroad, they should have registered your birth with the country’s U.S. embassy or consulate. If they did, they would have received a Consular Report of Birth Abroad . You can get a copy of this report from the U.S. Department of State. Depending on the country, a vital records office in the nation may also list the birth.
What if the State Department cannot locate your CRBA? If you were born on a military base abroad, your parents might not have registered your birth with the U.S. embassy. In that case, you may have to contact the hospital where you were born.
What Death Products Can I Get
Our office has death records from July 1, 1907 to present for events that occurred in the state of Washington. You can obtain Washington state death records as certified copies for legal purposes or noncertified informational copies for genealogical or other personal use. The four different death record products include:

Who Prepares The Death Certificate
The funeral home, cremation organization, or other person in charge of the deceased person’s remains will prepare and file the death certificate. Preparing the certificate involves gathering personal information from family members and obtaining the signature of a doctor, medical examiner, or coroner. The process must be completed quicklywithin three to ten days, depending on state law.

Obtain An Original Birth Certificate
When an adoption is finalized, the state issues a new birth certificate to the adoptive parents. The adoptees original birth certificate is then sealed and kept confidential by the states vital records department. Half the states in the U.S. require a court order to unseal an original birth certificate. However, many states allow access to original birth certificates through:
